Your first visit to a dermatologist is important, and it will be much easier for both you and our office if you are well-prepared.
New Patient Paperwork
All new patients are required to complete the following paperwork prior to their first appointment. If you were a patient with any of our providers at their previous practices, you are considered a new patient to Precision Dermatology and Skin Surgery and will need to complete the new patient paperwork.

Documentation to Provide at Your First Visit
Please bring the following items with you for your first appointment
- Driver’s license
- Insurance card(s)
- List of current medications (including strength, dosage, and frequency)
Patient Instructions
Wound care sheets are now available for patients and their caregivers to download online. If you are unsure as to which wound care sheet is associated with the procedure your physician performed, please contact our office at 904-923-6647. A member of our nursing staff will be happy to assist you.
